Match Penalty Reporting Policy
When reporting, the official must complete the NYSAHA Incident Report which must be completed and returned (postmarked), along with the origional scoresheet (no copies), within 48 hours of receipt.
FAILURE TO FOLLOW THIS POLICY MAY RESULT IN THE SUSPENSION OF THE OFFICIAL’S CERTIFICATION.

When a Match Penalty is assessed for an alleged PHYSICAL ASSAULT OF AN OFFICIAL (RULE 601 (G) 1 OR 601 (J) 1), you MUST CONTACT the New York State Referee-in-Chief IMMEDIATELY (in addition to reporting it to the Local Supervisor as stated above).
Game Official Responsibilities
- Advise player’s head coach immediately following the assessment of penalty.
- Upon completion of the game, the official will make notations in the remarks area of score sheet and cite match penalty number as per USA Hockey rule book.
- Keep the original or first copy of score sheet (white page) for further expedition. (Regardless of what the scorekeeper/coach/league president requests), you must keep the top/first copy to send in to the Local Supervisor. Do not allow the team/league to keep the copy
- Contact Section Local Supervisor assigned to area to verbally report incident on the same day penalty is accessed. This report should contain the following information player name, age classification, player’s association, location of rink, type of infraction.
- File a Match Penalty Incident Report via electronic mail (scan filled out form and e-mail) to Section Local Supervisor within 48 hours.
- Mail a signed copy of the Match Penalty Incident report and the original of first copy of score sheet (white page) to Section Referee in Chief.
